Abstract

This is a conference abstract from the "Psychosomatic Medicine Of The XXI Century: Realities And Perspectives" II Congress. It is dedicated to the sexual features of depressive disorders in adolescents and young adults.

Background

According to the WHO, more than 264 million people of all ages in the world suffer from depressive disorders. Depression is different from the usual mood swings and short-term emotional reactions to everyday problems. Depressive and anxiety disorders affect more than a third of the population each year. Depression causes a number of social dysfunctions that are more severe than chronic somatic diseases. [Goodwin F.K., Jamison K.R.]

The worst scenario for depression is suicide. Approximately 70% of patients with depressive disorders show suicidal ideation, and 10-15% of people with depression end their lives by suicide. [I.Y. Gurovych, E.B.Lyubov, S.A. Chapurin et al.]

Aim

The purpose of this study is to improve the differential diagnosis of depressive disorders in adolescents and young adults against the background of the study of their sexual characteristics.

Tasks

Tasks:

  1. To conduct a comparative analysis of clinical-psychopathological and clinical-pathogenetic features of depressive disorders (DR) in adolescence and young age (AYA) depending on gender.
  2. To determine the specificity of predictors of the severity of the course of DR in persons AYA depending on gender.
  3. To investigate the influence of self-destructive behavior on the course of DR in patients with AYA.
  4. To develop, scientifically substantiate and implement in clinical practice a screening method for predicting the severity of DR in women and men of AYA.

Materials and methods

According to the research plan, the following methods are used:

  • Hamilton scale for assessing depression (Hamilton, M., 1960),
  • Determination of autoaggressive predictors (Pylyagina G. Ya., 2004),
  • Beck's hopelessness scale (Beck A., 1974),
  • PHQ-9 patient health assessment scale (Kroenke K, Spitzer RL, Williams JB. 2001),
  • Five-factor personality questionnaire Big 5 (Heijiro Tsuji, translated by AB Khromov),
  • Test of Leontiev's meaning-life orientations (Leontiev D., 2000),
  • Assessment of aggression, anxiety, frequency of aggression, and anxiety on the subscales of the 8-factor Spielberger-Radyuk personality questionnaire
  • PHQ-15 patient health assessment scale (Kroenke K, Spitzer RL, Williams JB., 2002),
  • Methods of diagnostics of coping mechanisms of Heim (E.Heim, 1988).

Results

Currently, the study involves 30 females and males from 18 to 30 years.

The level of DR and the severity of somatic symptoms in the structure of DR were determined.

At this stage of the study, somatic manifestations of DR were determined among both women and men of AYA.

The distribution of the severity of depressive manifestations according to gender was assessed using the PHQ-9 patient health assessment scale and had the following form: severe DR was observed in the majority (56%) among women, moderate (33%) and severe (38%) DR prevailed among men.

The distribution of the severity of somatic manifestations of DR depending on gender was determined using the PHQ-15 patient health assessment scale and had the following characteristics: there were mostly severe (64%) and moderate (34%) somatic manifestations in the structure of DR in women. In men, a different picture was observed: the latter mostly had mild (27%), moderate (23%) somatic manifestations or no somatic symptoms in DR at all (37%).

Conclusion

At present, such results indicate the predominance in women of AYA of a heavier somatic component in the structure of DR in contrast to men of the same age category.
